Targeted Case Management

Targeted case management provides assistance to children and adults with developmental disabilities in gaining access to medical, social, educational, and other needed services. This service assists the person and their support network to identify, select, obtain, coordinate, and use both paid services and natural supports consistent with the person’s capabilities and preferences outlined in the Person-Centered Support Plan. The service includes assessing the person’s needs and preferred lifestyle, developing the individual’s Person-Centered Support Plan, and performing activities that help connect the person with medical, social, educational, and other needed services and/or resources to enable the individual to live their preferred life.

Each case manager must meet standards established by the state of Kansas.
